I took that as a natural consequence of a long-running campaign - a DM may be  reluctant to take out a character which a player - or indeed, the party - has a lot invested in. So they'll find ways to work a salvation into the storyline.

 

Unless you're playing Paranoia, natch.

nothing quite so machiavellian, it is actually hard to die in D&D.

 

I think that the instances you refer to where they appear dead but aren't are reflecting situations in game where a character is reduced to zero hit points which puts them unconscious. At this point in D&D you make "death saving throws" and you get 3 (more complex than that) and then you are dead (but even then you can be revived with a spell or two) so the party get at least 3 chances to lift your HP above zero.

Yeah, they do fail. Quite a few times, in fact. They fail a lot (a byproduct of them being a bit shit at planning and then the plan they do make going out the window almost immediately) and a few of their characters die permanently.
